The rising number of hospitalizations and overcrowding is growing exponentially every year in Ireland. These hospitalizations impose a lot of risks for those vulnerable people who are already immune-compromised by further exposing them to hospital-acquired infections (HCAI) and Covid 19 virus.

 

The HSE healthcare system has remained under very significant pressure for many years. This is causing a lot of financial pressures and valuable resources to be overstretched with rising labour costs, lack of beds, and staff shortages. The nursing project was implemented in a 135 bedded private care facility in the north Dublin area. It is a contemporary and ambitious project undertaken first by a private care facility.

The work breakdown structure and dictionary format were provided in a comprehensive structure based on a series of tasks required to complete the essential elements of the project. Project schedule, critical path, earned value management, key milestone lists, project organizational and resource structures, RACI Charts, communication, power/interest grid, status meetings, sample status reporting, project processes, and documents were provided to ensure support for the overall success of the project.

The project management plan of Hamilton Park Care Facility has put a formal process and system to facilitate the administration of first-line intravenous antibiotic therapy to residents in Hamilton Park Care Facility. This will promote the health and well-being of the residents, reduce hospital transfer, will increase bed availability in hospitals, and will provide a huge amount of huge savings for the HSE.

Finalist Special Recognition Award for Operational Agility COVID-19 Impact: Hamilton Park Care Facility

Hamilton Park’s strong enabling environment, including our effective clinical management team, care pathways, and models contributed to our success in preventing the transmission of Covid-19 Hamilton Parks Covid-19 Contingency Plans and Infection prevention protocols facilitated rapid response to the pandemic. With the early implementation of our Rapid Antigen Testing in April 2020 which provides a high level of testing and effective containment strategy using our infection control procedures, we have prevented major outbreaks and zero (0) Covid deaths in the care facility. Hamilton Park is the first large-sized private care facility in Ireland who have adapted and implemented large-scale Rapid Covid-19 Antigen Testing.

Finalist (1) Healthcare Initiative / Project Community Care Services: Hamilton Park Care Facility Rapid Antigen and Covid-19 Antibody Testing

Hamilton Park in 2020, arguably is the first nursing home/long-term facility to adopt the full implementation of Rapid Covid-19 Antigen Testing in Ireland. This is long before the HSE Covid-19 Antigen Test Validation Report in 2021, the NPHET Policy and Topic Advice for Covid-19 Antigen testing in August 2021, the HIQA recommendation of Rapid Antigen Testing of Asymptomatic Individuals in September 2021 and HSE Covid Antigen Test recommendations in December 2021.

This project provides insights in relation to the statistics of overcrowding in hospitals which were widely published and discussed on live radio, tv, and social media. This initiative has a huge potential in solving the vast ramifications to the lives of the people and the growing problem every year which costs a lot of resources and challenges to HSE and the health care industry.

Finalist Healthcare Initiative / Nursing Project of the Year: Hamilton Park Care Facility - Reducing Resident Hospital Transfer Resident thru Administration of First line IV Antibiotics

This initiative has promoted a safe transition of residents/patient care to a community setting. Residents’ unnecessary medical transfers to the hospital were avoided. This initiative has forecasted a monumental clinical outcome of improving residents’ well-being by removing the stress of staying in the hospital, promoting antibiotic stewardship, reducing the risk of acquiring infection from hospitals, providing treatment close to family support and where residents can sleep and eat better in a familiar comfortable environment. This plan is forecasted to free up hospital beds, thereby reducing the number of people placed on trolleys and increasing the overall hospital performance.

Finalist (1) Healthcare Initiative / Project Older Persons Care Services: Hamilton Park Care Facility - Reducing Resident Hospital Transfer Resident thru Administration of First line IV Antibiotics

It is a fact that older people living in nursing homes are frail with multiple chronic debilitating illnesses. Their high levels of cognitive and functional impairment make them vulnerable and prone to hospitalizations. Hamilton Park Care Facility implemented a Provider Led Initiative based on residents being initially admitted to the hospitals for IV antimicrobial treatment and then returned to the care facility. For this reason, David Pratt (CEO)  has decided to look at and enlist the support of the clinical team of Hamilton Park and HSE to improve the current situation we are facing in the healthcare setting.

Finalist (2) Healthcare Initiative / Project Older Persons Care Services: Hamilton Park Care Facility Rapid Antigen and Covid-19 Antibody Testing.

This early strategic plan was underpinned in 2020 based on the need for coronavirus testing in Ireland. The issue of speed and accuracy in the diagnosis of Covid 19 at that time poses a huge challenge for public health authorities. The clinical team of Hamilton Park understand that managing and detecting COVID-19 is crucial and that antibody testing is seen as a pillar in counteracting the spread of the virus. Hamilton Park has successfully demonstrated effective strategies together with the infection control procedures in preventing Covid-19 transmission which resulted in zero death.

Hamilton Park was well managed by an established management team who were focused on improving residents’ well-being and life in Hamilton Park. There were effective management structures in place that ensured care was provided in a safe and sustainable way. There were management systems and processes in place to promote the service and ensure that the service is safe, consistent, and appropriate to residents' needs.
 

The facility is continuously undertaking health service reviews for all the residents in the facility.  It is underpinned by the principle of continuity of care influenced by clinical governance that supports valuing and promoting residents’ health with the members of the multi-disciplinary team.
 

Our staff training programs and various staff development portfolios allow us to upskill the knowledge and competencies of our staff to better safeguard and well-being of the residents in the care facility.
 

Hamilton Park has implemented significant quality service improvement plans and maintained a high level of communication with our residents and counterparts both in the private and public sectors.
 

Hamilton Park has demonstrated elements of success across different phases of preparedness and response throughout the Covid-19 pandemic.

Finalist Nursing Project / Initiative of the Year: Hamilton Park’s 3-Tier Clinical Auditing System

A robust Quality Management Service Improvement Plan for clinical auditing was developed in measuring the quality of care and standards provided to the residents in the facility. The outcome of the clinical auditing system provided comprehensive compliance action plans and recommendations to improve residents’ safety and well-being for 2017, 2018 and 2019. Residents’ incidence of falls was reduced, the incidence of pressure sore was reduced and management pressure management improves, managing residents with responsive behaviours improves thru efficient assessments, reviews and care planning, risk and incidence of absconsion were reduced, and the use of inappropriate psychotropic medications were reduced, a dramatic decrease in the use of restraint and usage of enabler, reduce risk in smoking and alcohol consumption and the development of comprehensive protocol thru assessments, reviews and care planning for the end of life across the facility.
